Lookup Sheets

Populate dropdowns, autofill fields, and build conditional dropdowns in Google Forms live from a Google Sheet β€” set up in plain English, no code. Read-only and always in sync.

Quick Answer

How do I pull Google Sheets data into a Google Form?

Use Formfacade's Lookup Sheets. Go to formfacade.com/lookup, click Try it free, and pick what you want to do β€” populate a dropdown's options, autofill related fields, build conditional (cascading) dropdowns, or connect with AI. Then connect your form, connect your Google Sheet(s), and describe the connection in plain English. Formfacade reads the sheet live (every 30 minutes or on each submission) and never modifies it.

Google Forms has no native way to pull data from a Google Sheet. Lookup Sheets adds exactly that: it connects form questions to columns in your sheet and reads them live, so options and autofilled fields always match your real data β€” product catalogs, price lists, student rosters, employee directories. Your sheet stays the single source of truth; Formfacade mirrors it to serve forms fast and never writes to it.

What you can do with your Sheets data

After Try it free, Formfacade asks "What do you want to do with your Google Sheets data?" β€” four tasks, all powered by the same setup flow. Each has its own guide:

The picker screen: Populate a dropdown from Google Sheets, Autofill fields from Google Sheets, Conditional dropdowns from your Sheets data, Connect Google Sheets to your form with AI
Pick what you want to do β€” each option leads into the same connect flow.
TaskWhat it doesGuide
Populate a dropdownFill a dropdown's options from a sheet column (deduplicated, sorted, or filtered).Populate a Dropdown
Autofill fieldsWhen a respondent picks a value, auto-populate related fields from the same row.Autofill Fields
Conditional dropdownsFilter the next dropdown by an earlier answer, and drop choices that are used up.Conditional Dropdowns

Every option is natural-language driven β€” at the Configure step you describe the connection in plain English rather than set it up by hand.

Set it up (shared by all four)

Whichever task you pick, the first three steps are the same. The Connect form screen's heading changes to match the task, but the flow is identical β€” then each guide above picks up at the Configure step.

Start at formfacade.com/lookup

Go to formfacade.com/lookup, click Try it free, and choose what you want to do with your Sheets data.

Connect your form

Paste your Google Form link (or click Browse to pick it from Drive) and click Next. Formfacade makes a copy of the form to add the lookup to.

Step 1 Connect form: Make a copy of Google Form, with a link field, Browse, and Next
Step 1 β€” connect the form by pasting its link or picking it from Drive.

Connect your Google Sheet(s)

Click Connect a Google Sheet to launch the spreadsheet picker and choose your sheet. Back on Pick connected sheets, select one or more (use + Connect another sheet to add more), then click Next.

Step 2 Connect Sheets: a Connect a Google Sheet button and an 'I've connected, refresh' link
Connect a Google Sheet launches the Drive spreadsheet picker.
Pick connected sheets: a selected sheet with its sync date, and a Connect another sheet link
Pick one or more connected sheets β€” Formfacade reads their columns.

Describe the connection β€” then follow your task's guide

On Configure, you describe the connection in plain English. This is where the four tasks differ. Open the guide for what you're building: Populate a Dropdown, Autofill Fields, Conditional Dropdowns, or Connect with AI.

Behavior & limits

These apply to every Lookup task:

BehaviorDetail
Sync cadenceEvery 30 minutes, or on every form submission
DirectionRead-only β€” Formfacade mirrors your data and never writes to your sheet
List sizeHundreds to thousands of options, with autocomplete
Multiple sheetsConnect more than one; different fields can read from different sheets
Field visibilityAutofilled fields can be editable, read-only, or hidden
Unknown valuesThe form shows an error if an entry isn't found in the sheet
Source of truthThe spreadsheet; edits there propagate to every connected form

Prefer to keep your native Google Form?

The Try it free path makes a Formfacade copy served from a Formfacade link. To keep your original Google Form instead, use Install addon on the landing page and describe your lookup in plain English from inside Google Forms.

Responses still go to Google

Lookup only controls what options appear and what gets autofilled. Submitted responses keep flowing to Google Forms and your linked Google Sheet exactly as described in How Responses Sync.
